B5 is the size the notebook already is
Cornell notes work best when the ruled page lives inside the notebook you actually use, not as loose sheets in a separate folder. Across much of Asia that notebook is B5, and a Letter or A4 Cornell template either has to be trimmed or sits awkwardly proud of everything else. Printing at B5 means the sheets file with the rest of the course.
Less width means a harder choice about the cue column
B5 is narrower than A4, so the cue column and the note area are competing for less space. Keep the cue column wide enough to write an actual question in, because a column that only fits a keyword turns the method back into ordinary note taking with a stripe down the side. If something has to give, take it from the note area, since that is the part you can continue on the next page.
ISO against JIS, again
This uses ISO B5 at 176 by 250 mm. Japanese notebooks are JIS B5 at 182 by 257 mm, about 6 mm wider and 7 mm taller. For loose practice sheets that gap does not matter. For inserts meant to sit inside a specific notebook it does, and the fix is to measure the page and enter it as a custom size rather than trusting the label.
Printing it accurately
This only measures correctly if your printer doesn't rescale the page. In the print dialog set Scale: 100%, not "Fit to page" or "Shrink oversized pages". If you're not sure it worked, print the calibration page, measure the ruler on it with a real ruler, and it will tell you exactly what your printer is doing to the page.
