Proportions adjust to the page
A4 is narrower and taller than Letter, so the cue column and summary block are scaled proportionally rather than kept at the American 2.5 and 2 inch conventions. The relationship between the three areas is what matters — a cue column roughly a third of the width, a summary of roughly a fifth of the height — not the absolute measurements.
Filing and binding
A4 fits standard European ring binders and lever-arch files without trimming. If you are punching for a two-hole binder, raise the margin to about 20 mm so the cue column is not compromised by the holes — the cue column is the narrowest area and the first to suffer.
The summary is the part that works
Whatever the paper size, the effectiveness of the method rests on writing the summary from memory shortly after the lecture, and on covering the notes and answering the cues later. The layout is a prompt to do those things. Without them it is just paper with a line down it.
Printing it accurately
This only measures correctly if your printer doesn't rescale the page. In the print dialog set Scale: 100% — not "Fit to page" or "Shrink oversized pages". If you're not sure it worked, print the calibration page, measure the ruler on it with a real ruler, and it will tell you exactly what your printer is doing to the page.
